Preparing for Your COPD Clinic Visit
What to Bring to Your Appointment
Please check with your insurance company when scheduling an appointment; you may need an insurance referral and/or a co-payment.
Prior to your visit please have your pulmonologist’s office fax us the following:
- Most recent clinic visit note, including current medication list
- Most recent pulmonary function test (PFTs), both pre- and post-bronchodilator and 6-minute walk
- Chest CT (send the CD and written report) (if available)
- ECHO (if available)
Please remember to bring with you:
- Insurance card
- List of medications
