Support for Complex Health Care Needs
At BIDMC, we put patients at the center of everything we do. As part of our commitment to offer the best possible care, we created the Case Management Program.
Led by physicians and registered nurses, the Case Management Program ensures consistent, appropriate and safe care and discharges. We have 34 nurse care managers (NCM) who offer services and support for patients who may have complex health care needs and require several resources to manage their health.
Our nurse care managers:
- Assess patient and family needs
- Consult with PCPs and specialists
- Collaborate with insurance-based case managers to develop a post-hospital treatment plan
An essential component of the Case Management Program is linking patients back to their community and local resources.
Our program is administered within the larger patient care services division and helps promote stronger working relationships among nursing and other patient care services.
The Case Management Program is staffed seven days a week, and includes dedicated coverage in the emergency department.
