BIDMC recommends you plan ahead and name a Health Care Proxy – someone to speak for you and ensure your wishes and preferences are always understood and respected – just in case you ever become seriously ill.
The BIDMC Health Care Proxy mobile app allows you to complete the Massachusetts health care proxy form electronically and share it with your loved ones and healthcare team at BIDMC.
To get started:
