To Obtain Medical Records from Beth Israel Deaconess Medical Center (BIDMC)

Medical records are available for 20 years after the patient's discharge or final treatment. To obtain a copy of a medical record, an authorization to release protected health information must be properly filled out.

Record Fees

Patient or Patient’s Legal Guardian
We reserve the right to charge a reasonable cost-based fee for producing and mailing the copies, pursuant to HIPAA 45 CFR,164.524. This rate depends on the size and delivery method of the records being requested. At no time will the cost-based fee exceed Massachusetts law (MGL Chapter 111; Section 70).
Any Other Party
The fee structure follows Massachusetts law (MGL Chapter 111; Section 70).

Medical Imaging Requests

To request copies of X-ray, MRI, nuclear medicine or other imaging, please follow the Department of Radiology instructions under the "Obtaining Copies of Your Images" section.

Birth, Death and Marriage Records

For certified copies, please contact the City of Boston, Registry Division.
