Follow-up is an important part of our program in order for you to maintain your new, healthy lifestyle. Please be sure to keep all scheduled appointments and get labs drawn in the timeframe requested by your team. If you are unable to make your follow-up appointment, call 617-667-2845 as soon as possible.
Tracking Your Food
You must bring your completed food record sheet with you to your six-month and one-year follow-up visits, so please print it out and follow the instructions on the sheet.
