To submit your "Healthy Is…" statement, you must be 18 years or older.
I authorize Beth Israel Deaconess Medical Center (BIDMC) to use and display this statement on their web site, BIDMC.ORG as they choose to use it. The authorization is of indefinite duration. I understand that anything I write here will appear as is, including a name (if I choose to give one) and any other identifiable information. I further understand that BIDMC has the right to remove any or all of my statement for any reason and without contacting me. I may revoke authorization to use this Healthy Is statement on BIDMC.ORG by written request to jelyons@bidmc.harvard.edu. The revocation will not apply to any uses or disclosures that have occurred pursuant to the authorization up to the time of revocation.
