The following request an appointment form is NOT for urgent appointment requests. Your request will be sent to a BIDMC representative who will contact you via phone during regular business hours to schedule an appointment.

If you are having medical emergency and are in need of immediate assistance, please call 911.

If you have previously been seen in the department you are requesting, please call your physician's office or use PatientSite to schedule your appointment online.  To look up a specific physician, please visit Find a Doctor.
