Occupational Health

In order for your application to be finalized, information is required to meet BIDMC requirements, which is based on the following: state and federal regulations; Joint Commission Standards; and Infection Control policies. Your application will not be processed until all required documentation is received and approved by Employee/Occupational Health. This information is necessary so that we may protect both our patient population and you from potential infections of public health significance.  Required forms will be made available to each trainee via New Innovations - the health system Residency Management System.  Post-match or post-commitment, you profile will be created in New Innovations, and checklists will be sent to you.  One will include the forms necessary for Occupational Health.

1.  You will download the forms from New Innovations.  Gather from your medical provider the official documentation of your immunizations as requested. 

2.  Accurately enter all information requested in this pre-placement form.

3.  Fill in employee information at the top of the form and answer all questions.

4.  Complete the forms and scan and upload the forms back into New Innovations.
