Making an Appointment
New Patients
To be considered for surgery you must meet the eligibility criteria and then complete a screening process. This process is a series of appointments in which you meet members of our team, including a nurse, a medical doctor, a dietitian, a social worker, and one of the surgeons. The screening process is to ensure that Weight Loss Surgery is the best solution for your health. To initiate the screening process start by:
- Attending a free information session and/or view the online Webinar.
- Checking your insurance policy for requirements and coverage of bariatric surgery. We have created a worksheet to assist you in communicating with your insurance provider.
- Completing the Screening Form. This form can be downloaded from the computer, obtained at an information meeting, or just call the Weight Loss Surgery Center for a packet to be sent. You can return the completed form in person, by email, by mail, or fax.
Once your form has been reviewed by the Weight Loss Surgery Team a member of our staff will contact you. If you are eligible for WLS your first visit will be scheduled with our Medical Director and one of our nurses. During this appointment the doctor will determine what other tests and appointments will be needed for further evaluation. Please keep in mind the screening process can last 2 to 6 months and may, in some cases, take a bit longer. However, the end result is a well-informed, happier and healthier patient.
Bring a Loved One and a Notebook
We encourage you to invite a family member or trusted friend to your first appointment. In fact, we welcome a loved one at most appointments to help you as you gather information about your health and treatment. Remember to bring a list of any questions you have for the Weight Loss Surgery team, and a notebook so you can jot down information.
