Photo ID Cards
The Beth Israel Deaconess Medical Center Public Safety Department issues photo ID cards to all employees, students/interns, and outside contractors/vendors. Temporary ID badges may also be issued to medical center visitors. Per medical center policy
all persons issued an ID must wear it visibly on their person at all times while on hospital property.
It is assumed that all personnel understand the responsibility of being issued ID/access cards. ID cards must not be loaned or transferred to another person and must be returned to Public Safety upon termination.
ID Requests
All ID requests are processed in the Public Safety office. In order to submit a request for a new card (or changes to an existing card) please download the ID card request form our download forms page. The form must be filled out in its entirety and brought to the Public Safety office, located on Farr 1. (New Employees should receive the form during the new employee orientation process.) In order to be processed the form must bear an original signature of your supervisor/manager. Photographs will then be taken and the card issued. Photographs may be taken and cards issued 24 hours a day, seven days a week.
Lost or Stolen ID Cards
Many ID cards provide access to sensitive areas of the medical center which contain valuable personal property, delicate and expensive equipment, or sensitive research and lab experiments. Because of this it is important that employees never loan their card to someone else or allow access to unauthorized individuals. If you suspect that your card has been lost or stolen, please report it to the BIDMC Public Safety Dept. Immediately!
Your old card will be deactivated and a new one issued. Please note that all individuals will be charged a $5 replacement fee for all lost cards. Payment can be made at the cashier's office located in either the Clinical Center West lobby or on Feldberg 2 and the receipt brought to the Public Safety office. Alternatively payment can be made in the Public Safety office (cash only) at the time the new card is issued.
