Ordering a Breast Pathology Consultation
Step 1: Complete Forms
Download and complete the
Consultation Form and
Payment Authorization Form.
Step 2: Mail Forms with Materials
Mail the completed Consultation Form, Payment Authorization Form and pathology report with slides and/or blocks to:

We strongly suggest sending completed forms and materials via courier or express mail, with signature or receipt required.
International Consultations
The Breast Pathology Consultation Service also provides consultation services to facilities and physicians located outside of the Unites States. In addition to the above instructions, we do require the pathology report be in English. A loose translation by the pathologist requesting the consultation is acceptable. Please also make sure to include an e-mail address for easier communication.
