Structure and Process to Coordinate Patient Care
Patient Care Delivery is the structure and process by which responsibilities for patient care are assigned and work is coordinated among members of the nusring staff.
Collaborative Practive Model
As with other other components of Professional Practice, it must not exist in isolation but be developed and nurtured in tandem with Professional Values and Professional Relationships.
We call this the collaborative practice model. It includes the nurse , the patient, the physician and all others working together toward common patient goals. It is the nurse working in a collegial and collaborative relationship with the patient and other health professionals to determine the healthcare needs of patients and families. It is defined by the roles we all fulfill.
Nurse Coordinator Role
For example, the Nurse Coordinator who functions within a Primary care team. He/she functions as a vital member of the team coordinating the nursing care for a group of patients, along with other equally accountable nurses. Systems designed to function at the unit level support the consistency of team, seamless transitions and coordination of care.
