Glasgow Coma Scale

Comas are rated according to the Glasgow Coma Scale (GCS), which assesses three different functions: eye opening, motor response, and verbal response. Scores can range from 15 to 3. A lower score indicates a person is less responsive. Scores are interpreted as follows:

• 15-13-mild brain injury
• 12-9-moderate brain injury
• 8 or less-a severe brain injury

4 = Spontaneous 5 = Normal conversation 6 = Normal
3 = To voice 4 = Disoriented conversation 5 = Localizes pain
2 = To pain 3 = words but not coherent 4 = Withdraws from pain
1 = None 2 = No words, only sounds 3 = Flexion posture (decorticate)
1 = None 2 = Extension posture (decerebrate)
1 = None
