Post Discharge Clinic
The Post Discharge Clinic at Beth Israel Deaconess Medical Center provides care to Health Care Associates patients shortly after discharge from either the hospital or the emergency room. Staffed by hospital-based doctors, we address outstanding or ongoing medical issues related to the recent hospitalization. This program helps ensure that the transition out of the hospital and back to your doctor is seamless.
Location
The clinic is located within HCA on the 6th floor of the Carl J. Shapiro Clinical Center.

Scheduling
Patients can be scheduled via the HCA Call Center at 617-667-9600.
Appropriate Patients for Referral
Patients with HCA primary care doctors, or who are establishing primary care at HCA.
