What should I do if I have to cancel an appointment?

Please make every effort to come for a scheduled appointment.  We require a minimum of 24 hours notice if your have to cancel an appointment.  Remember, last minute cancellations limit our ability to serve other patients and increase waiting times for scheduled appointments.

What should I bring to my first appointment?

For your first appointment, bring a list of all medications you are currently using.  Always bring a copy of your current insurance card or cards for our billing records.

How can I change my primary care physician?

You can switch your primary care physician by contacting your health plan, usually by calling the telephone number on the front of your insurance card.

I am an established patient.  Why do you ask for my insurance card on every visit?

We need to ensure our records reflect any changes you may have made in your health insurance coverage.  If possible, we will verify eligibility prior to your visit.

Why are you asking for my co-payment at the time of my visit?

We request payment of co-payments at the time of your visit so you will not be bothered with an invoice several weeks after your visit today.  In addition, it helps us reduce our costs and saves you the inconvenience of mailing a payment back to us.

What should I do if I change insurance plans?

Please contact us as soon as possible.  Many health plans have strict filing limits, therefore any delay in receiving accurate insurance information may result in denied payment from an insurance plan.  Please see our office staff if you have any questions.

Will my health plan pay for physicals or wellness visits?

Some health plans, such as Medicare do not pay for annual physicals.  You should check with your health plan, insurance agent or employer to see if your insurance carrier pays for physicals and wellness visits.

I received a check from my insurance company for a medical visit.
