Please fill out and submit our
online request form to schedule
routine or non-urgent appointments, including bone density tests.
Your request will be sent to a staff member who will contact you via phone during our regular business hours (Monday - Friday, 8:30 a.m. - 5 p.m.) to schedule your appointment.
This form is not for urgent appointments or emergency situations. If you are in need of immediate assistance, please call 911.
