What is a Medical Home?

Your MEDICAL HOME is the place where you receive your health care- such as Bowdoin Street Health Center.  

You can expect that all of your health care needs are coordinated by your medical home team: physicals, sick or urgent visits, cancer screenings, diabetes or asthma care management, referrals to nutrition or counseling and other specialtieshome care, community resource specialists or whatever services you may need.

You know who your primary care provider is and who your medical home team is -and they know that you are part of their team. 

You  see your primary care provider/team for most of your health care visits.

Your MEDICAL HOME is TEAM- oriented with many individuals working with you and your primary care provider to help you meet your health care goals. 

You  have access to your medical home team before, during and after your health center visits either by phone or through Patient Site a secure email system for communicating with any member of your team.

What makes a medical home different from any other doctor's office?

In a Medical Home:

YOU- the patient - are the most important member of your medical home team.  

YOU and your medical home team, TOGETHER, develop a health care plan with goals that you can reach.

YOU are informed about and included in decisions made about your health care.

YOU are encouraged to ask questions and discuss what is most important to you.

How to get ready for your appointment:

Write down all of your health questions- with the most important questions first.

Make a list of all of the other health care providers you have visited.  Write down their names, addresses and phone numbers and the reason why you visited them.

Bring all of your medications, in their original containers, to your appointment.  Be sure to include all vitamins, over the counter medications, herbal medicines and prescription medications.

Bring your insurance card with you.

Ask a friend or family member to help prepare your questions and to go to your appointment with you.

Before you leave your visitmake sure your most important questions have been answered. Review with your medical home team, the health care plan you made together.   Ask questions and take notes if needed.
