Case Management
The case management program at Beth Israel Deaconess Medical Center is a nursing model organized by patient care units. Under the leadership of a physician dIirector, an R.N. director, an R.N. program leader, and an R.N. educator. Case management is administered within the larger Patient Care Services division. This alignment has promoted stronger working relationships among nursing and other patient care services disciplines.
Our 34 nurse case managers (NCMs) are responsible for coordinating the discharge planning of Beth Israel Deaconess inpatients. They employ Interqual criteria as a tool for utilization review and determining levels of care. Physician leaders partner with NCMs, take daily calls, consult in challenging situations, and problem-solve with the medical staff. The medical center’s case management program is staffed seven days a week, and includes dedicated coverage in the emergency department.
Beth Israel Deaconess is committed to a case management program that promotes consistency and appropriateness of care, and ensures safe and clinically appropriate discharge. The NCM assesses patient and family needs, consults with attending and primary care physicians, and collaborates with insurance-based case managers to develop a post-hospital treatment plan. An essential component of the program is linking patients back to their community, primary care physician and local resources.
